$270 yearly

Additional adult: 10% off plan fee.

all dental payment plans include:

All routine exams
All routine x-rays with exams
All problem-focused exams
All emergency x-rays as needed and when necessary as part of other services

20% off all regular cleaning or periodontal therapy/maintenance
20% off all restorative, surgical, and cosmetic treatments
$500 off SureSmile®

For additional adults on the same plan, eligible household members are defined as spouse, domestic partner, or child aged 14 years or older that live at the same physical address.

Children are defined as through age 13.
